Credex Healthcare LLC (“Credex,” “we,” “us”) provides medical billing, revenue cycle management, credentialing, payer enrollment, and licensure services to healthcare providers. 1. Scope; Patient Health Information

This Policy does not govern protected health information (“PHI”) that we receive from our clients’ patients’ records in the course of providing services. We handle PHI only as a business associate of our healthcare provider clients, under HIPAA and written business associate agreements with those clients. • Provider and client information: when we provide credentialing, enrollment, or licensure services, we collect information about healthcare providers such as license, NPI, DEA, education, work history, Social Security number, and date of birth, as needed to perform the services.
• Automatically collected information: IP address, browser and device type, pages viewed, referring pages, and approximate location derived from IP address, collected through cookies and similar technologies.

3. How We Use Information

We use personal information to respond to inquiries; provide, manage, and bill for our services; prepare and submit applications on providers’ behalf; communicate with clients; operate, secure, and improve our website; send service and marketing communications (you may opt out at any time); and comply with legal obligations.
