DME suppliers don’t just fight denials. They also handle documentation. If there isn’t a standard written order, proof of delivery that isn’t signed, or a rental month paid the wrong way, the claim may not be valid. That’s why business owners look for DME billing companies instead of just hiring a general biller. We included ours in this list of ten hired options that you can use to find a seller that fits your tools, payers, and software.

Just to be clear, the list includes external DME billing services, which are teams that do the work for you. These services cover billing for durable medical equipment (DME), orthotics and prosthetics (O&P), oxygen, CPAP, mobility equipment, and medical goods. Software systems have a short section of their own further down.

Key Takeaways

  • What to look for: Coders who have been trained in DMEPOS, rental tracking with a cap, record checks before claims are sent out, and monthly reports on denials and days in arrears.
  • Typical pricing: Most companies charge a share of the money they collect, and not many of them publish a rate. You can expect to be asked for a price.
  • Why enrollment matters: A provider can’t bill Medicare until it is approved, bonded, and enrolled through the CMS-855S. Find out who manages this process.
  • Credex Healthcare is best for suppliers that want billing and Medicare enrollment support from one team.

Integra Global Solutions says that it has been doing RCM work since 2004. Its website says it can bill Medicare, Medicaid, managed care, and commercial insurance, and it works directly with DME companies. As part of the services, medical coding and reviewing patients’ credit balances with secondary bills are provided.

Key DME billing services include coding for medications, claims, and additional bills; reviewing patient credit amounts; and following each payer’s billing rules. The company is best for providers who work with a lot of Medicare and Medicaid patients.

TRANSCURE is a San Diego company that has been around since 2002 and says it works with over 1,000 providers in over 40 different fields. Its website says AAPC-certified coders handle CPT, HCPCS, ICD-10, and modifiers. It also says that DME billing is part of its podiatry work.

Key services from DME billing include evaluating HCPCS codes and modifiers. They also verify eligibility and prior authorization and manage claims and denials. The company also offers medical credentialing services for DME practices. It is best for suppliers who want to automate their processes and work with just one provider for a variety of services.

MedKloud

MedKloud says it helps growing practices with billing, credentialing, and marketing. Its website highlights medical billing, billing checks, RCM services, and real-time claim tracking and reporting.

Key services for DME practices include auditing medical billing, tracking claims, credentialing, and reviewing data and reports. Its services are best for suppliers that want an audit of current billing before they outsource.

MyOutDesk

my out desk logo

MyOutDesk hires virtual helpers who are experts in medical billing. Their job is to enter claims, post payments, handle denials and accounts receivable, check eligibility, and get prior authorizations in your systems. On its page, it says its billing helpers aren’t qualified hackers and cost up to 70% less than an in-house biller at the same level.

Key DME services include claims entry and submission, payment posting, denial and AR work, eligibility checks, and prior authorizations. The company seems best suited for suppliers that keep billing management in-house and need capacity.

What Do DME Billing Services Include?

A full service runs the revenue cycle from the first benefits check to the final payment. Here’s what to expect:

  • Verification of benefits and eligibility before equipment goes
  • Prior approval for things on CMS’s list of things that are needed
  • Billing and coding for DME using HCPCS Level II codes, modifiers (like rental, purchase, left/right, and KX), and units of service
  • Cleaning up claims and sending them through a clearinghouse
  • ERA files are used to post payments
  • Keep track of rental and refill costs
  • Handling denials and following up on AR
  • Reports every month on the number of clean claims and days in arrears, as well as patient responsibility statements

Coding errors sit behind many denials, so some suppliers add separate medical coding services.

Why DME Billing Is Harder Than General Medical Billing

Start with the codes. DME bills under HCPCS Level II, not CPT. The same item can be billed as either a rental or a purchase. Changes like RR, NU, KX, GA, GZ, and LT or RT tell the payer which.

Rentals have a calendar. Medicare will cover renting certain items once a month for up to 13 months, and you can rent breathing tools for up to 36 months. You lose money if you miss a month.

Then there is the paperwork. For a claim to be valid, you need a standard written order and proof of delivery, and the records must meet LCD and policy requirements. Some items also need to be discussed in person or include a Certificate of Medical Necessity (CMN). Things on CMS’s list also need to be approved ahead of time.

There are always audits. An Additional Documentation Request (ADR) can be sent by a DME MAC or RAC reviewer, and records that are missing mean that the money must be paid back. Competitive bidding is also back. CMS’s rule for CY 2026 brought it back, and new contracts will start in January 2028.

That makes DME medical billing a unique skill, and it’s even harder to do DMEPOS billing in more than one place. HME billing services follow the same rules as DME billing services because they are both billed in the same way. Commercial and Medicaid payers each have their own rules.

Medicare DME Billing Starts with Enrollment

When you sign up for Medicare DME, billing begins, not with claims. For each NPI, a supplier needs to get approval from an organization that CMS has approved and post a $50,000 surety bond. Then, it sends the CMS-855S to a National Provider Enrollment (NPE) contractor, either through PECOS or on paper, with the Name of the Provider (East) or Palmetto GBA (West).

It’s not the end when you get approval. CMS can revoke your ability to bill if the approval or the bond expires, and revalidation happens on its own schedule. CMS’s CY 2026 rule also changed DMEPOS approval to once a year, so check with your accreditor to make sure you know the latest rules.

That’s where your billing partner matters, because a biller that only files claims leaves enrollment to you. Credex Healthcare pairs billing with DMEPOS enrollment support, and BillingParadise lists supplier number enrollment. Our guide to the best credentialing companies for medical equipment providers compares more.

Outsourced Billing vs. DME Billing Software: Which Do You Need?

Software is something that your staff manages. A billing service is a group of people who do the work for you, usually using your software.

Platforms like Brightree, Kareo, CareCloud, and AdvancedMD take care of registration, ticket delivery, and claims. The claims are then sent to payers by the system. Some of the above sellers, like Medbill and MedCare MSO, offer both.

If you have trained billers working for you, choose software. If you don’t, or if rejects keep coming in, pick a service. You don’t have to move systems if you want to integrate with an EHR. It’s just something you should think about.

How Much Do DME Billing Services Cost?

Most companies charge a share of the money they get. Some charge a flat monthly fee or a fee per claim. One company on this list doesn’t give an exact rate. Some of the above companies also offer EHR or office management services, so make sure you know what’s included. The people don’t know much. Full-cycle RCM costs 5% of collections from Transcure, a percentage of collections from BillingParadise, and up to 70% less than an in-house biller from MyOutDesk.

To give you an idea of the numbers, a 6% fee on $2 million in yearly collections comes to $120,000. That’s not even close to the pay, perks, and change an in-house biller gets. See our guide to the best billing services for small practices for more information on how to find the best one for your business.

How to Choose the Right DME Billing Company

Before you sign, compare any vendor to these seven things:

  • Specialty Check: Ask the DME who works on your account how many rental accounts they are in charge of.
  • Checks on Paperwork: Find out how they make sure the standard written order, CMN, and proof of delivery are correct before they bill you.
  • Help with Enrollment: Find out who keeps track of the surety bond, accreditation, and revalidation.
  • Audit Support: Ask the audit support team who answers an ADR or RAC request and how quickly they do so.
  • Software Compatibility: Make sure that your EHR works with your platform.
  • Reporting: Ask for an example report that breaks down the number of clean claims, rejections, and days in arrears by product type.
  • Contract Terms: Find out about HIPAA compliance, a business associate agreement, and how to get out of the contract.

Smaller suppliers should check billing for small practices, since minimum volumes differ.

Outsource or Keep DME Billing In-House?

When you don’t have enough trained billers, your rejections are going up, or your rental months are falling, it makes sense to outsource your DME billing. Stay in-house if you have a strong team with deep knowledge.

Many sellers split the work: they handle intake and delivery tickets themselves and work with a partner to handle claims and accounts receivable follow-up. Make sure you get reports that you can read either way.

Signs It’s Time to Switch DME Billing Companies

Switching vendors is a hassle. Staying put costs more when you see these signs:

  • DME claim denials keep climbing, especially for CMN or modifier errors
  • Capped rental months get missed or billed out of order
  • AR over 60 days keeps growing
  • Audit letters arrive, and nobody owns the response
  • You get no reports by product category
  • Emails sit unanswered for days

A medical billing audit shows where claims leak before you decide.

FAQs

What do DME billing companies do?

They make sure that equipment sources get paid. They check benefits, review delivery orders and proof of delivery, code and submit claims, post payments, handle denials, and follow up on unpaid accounts receivable. Some also help people sign up for Medicare and send monthly reports on collections and days in arrears.

How much do DME billing services cost?

Some charge a flat monthly or per-claim fee, but most charge a portion of what they get paid. One company, Transcure, says that full-cycle RCM costs 5% of receipts. The final cost depends on the number of claims, the type of equipment used, and the size of the job. Get written quotes.

What do I need to enroll as a Medicare DME supplier?

You must be cleared by CMS, have a $50,000 surety bond for each NPI, and complete the CMS-855S application. You can do this online or on paper. Palmetto GBA oversees NPE West, and Novitas oversees NPE East. Some professionals don’t have to be accredited.

How does capped rental billing work?

Medicare covers the rental of certain items for up to 13 months each month. After that, the patient becomes the owner of the items. You need the RR suffix and the correct date of service every month. Oxygen equipment has a separate 36-month rental period, so it’s important to track it.

Should I outsource DME billing or keep it in-house?

If you don’t have enough trained DME billers, if your denials are going up, or if your rental months are falling, you should outsource. If you have a good biller and good tools, keep it in-house. Many sellers split the work, keeping the processing process in-house and hiring outside help for follow-up on claims and AR.

Can a DME billing company also handle Medicare enrollment?

Some can, so ask before signing. Credex Healthcare covers billing, DMEPOS accreditation checks, Medicare supplier number maintenance, and Medicaid and commercial payer credentialing. A lapse can stop Medicare payment. Credex Healthcare handles both together; book a free consultation.

What’s the difference between DME billing software and a billing company?

Software is something that your staff uses. A billing company is a group of people who do the work, usually using your software. When you buy something, make sure you know whether it’s a tool, a team, or a bundle.

How do I switch to DME billing companies without losing revenue?

Plan the handover around open AR, live authorizations, and open AR. Set a date for the switchover, share rental schedules and payer lists, and have your old vendor finish up any open claims. Credex Healthcare can help you plan the transition during a free meeting.

Choosing the Right DME Billing Partner

The best DME billing companies all share three things: coders trained in DMEPOS, thorough checks of all paperwork, and easy-to-read reports. That should match your software and mix of payers. Then you should ask for examples.
